I quoted the part of the "Part 1 Instruction" I didn't understand.
I made a custom_email_xx.tpl file but what folder does it go? And what file do I have to modify to direct to that file?

This is a drop-in, if you wish to make special email templates for specific usergroups, e.g. newsletters
## this is done by making a new email template (to all installed langauges) and name it
## custom_email_XX.tpl, where XX is substituded with the usergroup name, if you witch to make custom templates
## for "all users", simply make a template with the name custom_email.tpl

you should place this newly created email template file inside every language folder

e.g.
language/lang_english/email/here it goes.

the XX mentioned in the how to should be speeled EXACTLY as the usergroup name - this will inshure, when ever you are using this script to mass email, that it will be used (if pressent in all languages)

so you do not need to modify any other files, the script will try use this new file if the name is correct..
